Invasive Epilepsy Monitoring: The Switch from Subdural Electrodes to Stereoelectroencephalography

نویسندگان

چکیده

Abstract Stereoelectroencephalography (SEEG) has experienced an explosion in use due to a shifting understanding of epileptic networks and wider application minimally invasive epilepsy surgery techniques. Both subdural electrode (SDE) monitoring SEEG serve important roles defining the epileptogenic zone, limiting functional deficits, formulating most effective surgical plan. Strengths include ability sample difficult reach, deep structures brain without craniotomy disrupting dura. is complementary treatment options may reduce gap patients who are hesitant about resection. Understanding strengths limitations SDE allows epileptologists choose best modality for each patient living with drug-resistant seizures.

Complications of invasive subdural grid monitoring in children with epilepsy.

OBJECT This study was performed to evaluate the complications of invasive subdural grid monitoring during epilepsy surgery in children. METHODS The authors retrospectively reviewed the records of 35 consecutive children with intractable localization-related epilepsy who underwent invasive video electroencephalography (EEG) with subdural grid electrodes at The Hospital for Sick Children betwee...

Complications to invasive epilepsy surgery workup with subdural and depth electrodes: a prospective population-based observational study

OBJECTIVE In some patients who undergo presurgical workup for drug-resistant epilepsy invasive seizure monitoring is needed to define the seizure onset zone and delineate eloquent cortex. Such procedures carry risks for complications causing permanent morbidity and even mortality. In this study, prospective data on complications in a national population-based sample were analysed. DESIGN Comp...

Evaluation of intractable epilepsy. Invasive monitoring.

Accurate localization of the epileptogenic zone is the fundamental key factor for successful epilepsy surgery. Despite the progress achieved in the field of neuroimaging, invasive intracranial recording is still the gold standard that helps recognize the patient population who may profit from surgery. Meticulous implantation of intracranial electrodes and judicious interpretation of their data ...

Difficult-to-Localize Intractable Focal Epilepsy: An "In-Depth" Look.

Commentary Monitoring with intracranial electrodes is the gold standard for identification of seizure-onset zones and mapping of eloquent cortex in patients with intractable focal epilepsy. Electrode implantation is guided by hypotheses about localization of the epileptogenic zones that are based on assessment of seizure semiology, interictal and ictal scalp-EEG, functional and anatomic neuroim...

Rapid and fully automated visualization of subdural electrodes in the presurgical evaluation of epilepsy patients.

For rapid visualization of subdural electrodes with respect to cortical and subcortical structures, we describe a novel and fully automated method based on coregistration, normalization, optional cerebellum masking, and volume rendering of 3D MR imaging data taken before and after implantation. The key step employs the skull-stripped preimplantation image as a mask to also remove the skull in t...
